- Pathological/medical reports related to the sickness
- Sick leave certificate
- Certified true copy of ID card
Note:
- You MUST submit the claim through the policyholder (i.e. your employer) or the Human Resources Department of the policyholder.
- Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.
- You are kindly reminded to make copy of your completed claim form and supporting documents for your records and future reference.
