Required supporting documents

  • Confidential Medical Certificate, completed by the attending doctor
  • Copies of histopathology and lab results related to the illness
  • Certified true copy of HKID Card
Note:
  1. You MUST submit the claim through the policyholder (i.e. your employer) or the Human Resources Department of the policyholder.
  2. Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.
  3. You are kindly reminded to make copy of your completed claim form and supporting documents for your records and future reference.
