Claim document checklist

If you are unsure about the claim benefit item, you can submit all claim documents to us.

Claim Form Part I and Part II (The form required on injury condition (accident / medical / dismemberment), click here to download a relevant form)
Proof of participation of the Covered Run Examples: registration payment proof, Runner's bib with insured's name
Proof of diagnosis
Copy of diagnostic imaging report Examples: x-ray, computed tomography, magnetic resonance imaging
Proof of treatment (and hospitalization, if any)
Proof of surgery (if any) Examples: discharge slip/summary with details of surgery performed
ID card copy A valid ID card copy is required if this has not been submitted previously

We may ask for other documents on a case by case basis.

If you submit the claim application through AIA financial planner / your broker / independent financial adviser, you will receive a notification from the respective channel.

If you submit the claim application to us directly, you will receive a notification from us.
