You may submit the claim application to our AIA financial planner / your broker / independent financial adviser or send it to us at the following address:

If you submit the claim application through AIA financial planner / your broker / independent financial adviser, you will receive a notification from the respective channel.
 
If you submit the claim application to us directly, you will receive a notification from us.

Once the claim is approved, you will receive the claim payment through the payment method you have chosen.
 
 Register FPS or e-Bankin via AIA Connect for receiving claims payment directly

Functional test report (and proof of treatment):
For heart failure: blood test report, chest x-ray report, electrocardiography (ECG), echocardiogram, treadmill report and cardiac nuclear medicine report
For liver failure: proof of permanent jaundice, ascites and hepatic encephalpathy
For kidney failure: renal function test report and proof of peritoneal dialysis / haemodialysis or renal transplantation
For lung function failure: forced expiratory volume (FEV) 1 test report, arterial blood gas analysis report and oxygen therapy recordFor heart failure: blood test report, chest x-ray report, electrocardiography (ECG), echocardiogram, echocardiography, treadmill report and cardiac nuclear medicine report
