Claim document checklist

if you are unsure about the claim benefit item, you can submit all claim documents to us.

Sick leave certificate
Proof of disability Examples: doctor's follow up assessment record, physiotherapy/ chiropractic treatment/ occupational therapy progress report
Dismemberment Claim Form
Proof of dismemberment Examples: surgery record, doctor's follow up assessment record, medical reports
ID card copy A valid ID card copy is required if this has not been submitted previously

We may ask for other documents on a case by case basis.

Note: Apart from claim form, you may submit other supporting documents in copies.

You may submit the claim application to our AIA financial planner / your broker / independent financial adviser or send it to us at the following address:

If you submit the claim application through AIA financial planner / your broker / independent financial adviser, you will receive a notification from the respective channel.

If you submit the claim application to us directly, you will receive a notification from us.
